What is Perianal Fistula?

The word fistula refers to the relationship between two or more surfaces or holes in the body. Anal or perianal fistula is a connection between the inside of the anus and the skin around the anus. That can be a simple, short-lived relationship, or a deep, complex relationship and a lot of detours. Dr. Hossein Yousef fam has been given a detailed explanation of the cause of perianal or anal fistula and its treatment.

Cause of perianal abscess and fistula :

Most fistulas usually start with an anal abscess and the abscess itself is caused by the opening of the anal opening. There are a few called cripti inside the anus whose function is to produce a slimy moon to easily pass the stool through the anal canal. When one of these glands closes, the secretions within the glands become infected, causing the pus to accumulate in the gland, which gradually enlarges and causes an abscess cavity. Inside which is the accumulation of pus.

The resulting abscess may spread to the perianal spaces and cause it to become a variety of perianal abscesses.

Absorption of the abscess either spontaneously or by the surgeon usually results in the formation of a pathway called the fistula, which is subdivided into different types depending on its depth and location.

Types of perianal fistula :

  • Fistula between sphincters
  • Trans-sphincter fistula
  • High sphincter fistula
  • Sphincter External Fistula

Perianal fistula detection methods :

A detailed history and examination of the fistula diagnose in 98% of cases and can be used to determine the depth of the endoannal or MRI scan. Fistulography has almost no place in fistula diagnosis and is not applicable today.

Perianal fistula treatment methods :

Almost all fistulas require surgery and there is no medication, traditional therapy, home remedies and herbal remedies for anal fistula.

Depending on the severity of the fistula, its depth, the complexity of the fistula, and the involvement of the sphincter muscles, different methods can be used for treatment. There is a possibility of recurrence of fistula in different surgical treatments of the fistula and this may vary depending on the surgical procedure.

Fistulotomy : Open the fistula duct

Fistulectomy : Removal and removal of the entire fistula duct

LIFT method : Closing the fistula through the sphincter

Adjuvant rectal flap procedure

VAAFT Method : Read more about the VAAFT technique performed by Dr. Yousef Fam for the first time in Iran three years ago at the VAAFT Fistula Surgery Link.

Fistula Columnation : The use of a column (a band or thread temporarily inserted into the fistula) is used in fistula surgery as a bridge between the first and second surgeries, and its role is to keep the fistula duct open so that it does not abscess again.

Column insertion is sometimes used as a treatment for fistulas, which is a long, difficult and painful treatment and is less commonly used today.

Fistula Laser Surgery : The fistula is a laser that can be treated with multiple, unpredictable side paths, and a laser can be used when there is a specific duct for the fistula.
